MOTOR INSURANCE CLAIM GUIDE

The following information and/or documents are necessary and shall be required for the
adjudication of claims

FOR OWN DAMAGE

THIRD PARTY CLAIM

/ TOTAL LOSS OR THEFT

1) Photocopy of Insurance Policy;
2) Proof of payment/official receipt of your insurance premium (both required for third

party claim

3) Endorsement, if any;
4) Photocopy of the Certificate of Registration of the insured unit ( both required for third

party claim

5) Photocopy of the Official Receipt of Registration ( both required for third party claim);
6) Photocopy of the Driver's License and Official Receipt ( both required for third party

claim

7) Stencil of motor number and serial/chassis number ( both required for third party

claim

8)        Police Report and/or duly notarized affidavit of Incident
9)        Repair Estimate of the damaged unit
10)        Pictures of the insured damaged unit, third party vehicles or properties involved
11)        Certificate of No Claim of third party vehicles
12)        In Case of Carnapping, Carnapping Report from Anti-Carnap Unit in Camp Crame
13)        Three (3) months recovery period is required before Replacement of Unit
14)        Client shall pay the Deductible amount due

FOR BODILY INJURY

Aside from the above documents the following is required
1)        Whereabouts of the insured unit, third party vehicles or victims, if any;
2)        Medical Certificates
3)        Receipts of Medicines, X-ray, etc.
